CoastConnect subscribers could be eligible for payment assistance through a federal program, Lifeline. Another federal program, the Affordable Connectivity Program (ACP), is set to end. Applications are no longer available. Existing ACP recipients may fill out our form for transfers only. Please read this statement about ACP.

You should only fill out this form if you are already enrolled in ACP and are transferring your benefit. ACP is now closed to new applicants.

4 The Affordable Connectivity Program (ACP) is a government program that reduces your broadband internet access service bill. You may apply the ACP benefit to any CoastConnect broadband service offering at the same terms available to households that are not eligible for ACP-supported service. You may obtain ACP-supported broadband service from any participating provider of your choosing, and you may transfer your ACP benefit to another provider at any time.

For Transfers Only: By signing this form, I acknowledge the following: (1) I will be transferring my ACP benefit to CoastConnect; (2) the effect of the transfer is that the ACP benefit will be applied to CoastConnect's service and will no longer be applied to service retained from my previous ACP provider; (3) I may be subject to my previous ACP provider's undiscounted rates as a result of the transfer if I choose to maintain that provider's broadband service; (4) my household is limited to one ACP-transfer transaction per service month, with the limited exceptions of reversing an improper transfer or addressing situations impacting my household's receipt of ACP-supported service from a particular provider.

The Affordable Connectivity Program (ACP) is a government program operated by the Federal Communications Commission. The ACP reduces customers’ bills for broadband internet access service and certain related equipment charges. The benefit is non-transferable, limited to one discount per household, and temporary in nature. If a customer chooses to continue to receive CoastConnect broadband service after the ACP ends, the customer will be subject to CoastConnect’s undiscounted rates and general terms and conditions.

Lifeline

CoastConnect subscribers could receive a monthly credit of up to $9.25 through Lifeline. To receive this benefit, follow these steps:

This signed Lifeline Enrollment Form ("Enrollment Form") is required to enroll you in CoastConnect's Lifeline program. The National Verifier, not CoastConnect, determines your eligibility to receive Lifeline. Only persons who have been determined to be eligible by the National Verifier should complete this Enrollment Form: If you have not qualified for Lifeline through the National Verifier, please visit https://nationalverifier.service-now.com/lifeline to apply or complete and submit your paper application. The information you enter on this Enrollment Form must be the same as what you provided to the National Verifier.

Lifeline is a government assistance program to reduce your phone and/or broadband internet bill, from service with any participating service provider of your choosing. You cannot transfer your service benefit to someone else and only eligible consumers may enroll in the program. The program is limited to one discount per household, which is defined as any individual or group of individuals who live together at the same address and share income and expenses.

If I qualify for another government assistance program (such as the Affordable Connectivity Program), I understand that I may choose to participate in both programs with CoastConnect, although I am not required to do so.

CoastConnect's general and Lifeline Terms and Conditions of service apply to existing or newly activated service (A copy may be obtained by visiting www.CoastConnect.com). CoastConnect offers Lifeline services only in areas where it has been designated as an Eligible Telecommunications carrier.
